What to expect
- You complete a medical questionnaire when you order.
- A licensed provider reviews your history, your answers, and your risk factors. They may approve you, decline, or come back with questions.
- If approved, your prescription goes to a licensed US compounding pharmacy, which fills it and ships it to you.
- Your provider chooses which medicine and which strength. It is not something you select, and it is not the same for everyone.
Two medicines are used to treat erectile dysfunction here. Tadalafil is supplied as a mango-flavored sublingual that dissolves under the tongue. Sildenafil is the other option. They differ in how quickly they take effect and how long they last, and your provider will explain how to take whichever one they prescribe.
Erectile dysfunction medication is dispensed as either a 10-day or a 30-day supply, depending on what your provider prescribes.
Safety and side effects
The notes below are drawn from FDA-approved labeling for sildenafil and tadalafil. Compounded medicines, including the sublingual form of tadalafil, are not approved or evaluated by the FDA, so this describes the active ingredients rather than this specific compounded product.
Never take this with nitrates. Combining these medicines with a nitrate can cause a sudden, dangerous drop in blood pressure. That includes nitroglycerin, isosorbide mononitrate and isosorbide dinitrate, and the recreational inhalants sold as poppers, such as amyl nitrite or butyl nitrite. It also includes riociguat, sold as Adempas.
If you get chest pain during or after sex, get emergency help and tell them you have taken this medicine, so that you are not given a nitrate.
Seek emergency care for an erection lasting more than four hours. This is called priapism. Left untreated it can cause permanent damage.
Stop and get medical help straight away if you have
- Sudden loss of vision in one or both eyes
- Sudden decrease or loss of hearing, sometimes with ringing in the ears or dizziness
- Swelling of the face, lips, tongue or throat, or trouble breathing
- Chest pain, or a severe or persistent dizzy or faint feeling
Commonly reported effects
- Headache, flushing of the face and neck
- Indigestion, heartburn, or an upset stomach
- Blocked or runny nose
- Dizziness or lightheadedness
- With sildenafil: changes in color vision, a blue tinge to what you see, blurred vision, or sensitivity to light
- With tadalafil: back pain and muscle aches, usually starting a day or so after a dose and settling within a couple of days
Before you start, tell your provider if you
- Take any nitrate medicine, riociguat, or use poppers
- Take alpha blockers such as tamsulosin or doxazosin, or other blood pressure medicines
- Take medicines for HIV, antifungals, or certain antibiotics, which can raise the level of these medicines in your blood
- Have heart disease, or have had a heart attack, stroke, or serious heart rhythm problem
- Have blood pressure that is uncontrolled, either high or low
- Have ever had sudden vision loss from reduced blood flow to the optic nerve, or have retinitis pigmentosa
- Have sickle cell anemia, leukaemia, or multiple myeloma, which raise the risk of priapism
- Have a curved penis, Peyronie disease, or any deformity of the penis
- Have severe liver or kidney problems, a stomach ulcer, or a bleeding disorder
Also worth knowing
- Alcohol can add to dizziness and low blood pressure
- Grapefruit and grapefruit juice can raise the level of these medicines in your blood
- Do not take more than your provider prescribes, and do not combine it with other erectile dysfunction medicines
- These medicines do not protect against sexually transmitted infections
