Semaglutide is a GLP-1 receptor agonist. It is compounded together with vitamin B12, which is why your vial is labeled semaglutide/B12. It may be used to support weight management and metabolic health, and is often discussed in relation to appetite regulation and blood sugar control. Whether it is appropriate for you is a decision a licensed provider makes after reviewing your medical history and your questionnaire answers. No lab work is required to be considered.
The three month option exists to cut down the back and forth. You are reviewed once and your medication is dispensed as a 90-day supply rather than as three separate monthly fills. The price does not change as your dose changes, and there is no membership fee on top of it.

Safety and side effects
The notes below are drawn from FDA-approved labeling for semaglutide. Compounded medicines are not approved or evaluated by the FDA, so this describes the active ingredient rather than this specific compounded product.
Thyroid tumor warning. In animal studies, medicines of this type caused thyroid tumors, including a cancer called medullary thyroid carcinoma. It is not known whether they do this in people.
Do not take this if you or anyone in your family has had medullary thyroid carcinoma, or if you have Multiple Endocrine Neoplasia syndrome type 2.
Tell your surgeon or anesthesiologist before any procedure involving sedation. These medicines slow how quickly your stomach empties, which raises the risk of food coming back up while you are sedated. You may be asked to stop beforehand.
Stop and get medical help straight away if you have
- Severe stomach pain that does not go away, with or without vomiting. This can be inflammation of the pancreas
- A lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath
- Swelling of the face, lips, tongue or throat, or trouble breathing
- Pain in the upper right of your stomach with fever, yellowing of the skin or eyes, or pale stools. This can be a gallbladder problem
- Vomiting or diarrhea bad enough that you cannot keep fluids down
- New or worsening changes to your eyesight
Commonly reported effects
- Nausea, vomiting, diarrhea, constipation or stomach pain, most often when starting or increasing a dose
- Heartburn, burping, or feeling full very quickly
- Tiredness, headache or dizziness
- Redness, soreness or swelling where you inject
Before you start, tell your provider if you
- Have ever had inflammation of the pancreas or gallbladder problems
- Have kidney or liver problems
- Have diabetes, especially if you take insulin or a sulfonylurea, because your blood sugar can drop too low
- Have diabetic eye disease
- Have a stomach condition that slows digestion
- Have had depression or thoughts of harming yourself
- Are pregnant, breastfeeding, or planning a pregnancy. This is normally stopped well before trying to conceive
If you are injecting at home
- Use a new needle and syringe every time, and never share them
- Put used needles straight into a sharps container, not household trash
- Rotate where you inject to reduce soreness
- Contact a provider if an injection site becomes increasingly red, warm, swollen or painful, or if you develop a fever
